Meeting held on the 26 October 2010 at Muirton Ward, Seafield Hospital, Buckie
Re: Planning of care and support before discharge from Hospital

Jessie Smith (27.09.41)

Attendees:

Graham Cameron - Social Worker (Chair)
Dr L Fourie - Locum Consultant in Old Age Psychiatry
Alison McPherson - Staff Nurse (Muirton Ward)
Ian Allan - Manager - (Whinneybank Day Care)
Sandra Stevenson - Care Worker (Whinneybank Day Care)
Linda Smith - Care Worker (Whinneybank Day Care)
Gaye Morrison - Adult Protection Co-ordinator
Katie Smith - Daughter (POA)
Fiona Smith - Daughter - Main Carer (POA)
Fran Gardiner - Daughter
Martin Smith - Grandson

Katrina Murray - Community Mental Health Nurse (Minutes)

Jessie was admitted to Muirton Ward six weeks ago.

Purpose for meeting: For Jessie to be discharged home with support for family and Jessie.

Discussed at meeting for carers in the morning, Abbeyvale Day Care Monday - Friday and to keep link with Whinneybank Day Care Saturday and Sunday. Fiona, Katie and Fran agreed to morning carers at this time and to day care arrangements.

Care package arrangements to be reviewed in 4 weeks time on Thursday 25 November 2010 at 10am at Pluscarden Clinic.

There would be an option for Jessie to return to Whinneybank 7/7 days should she not settle at Abbeyvale.

Fiona was offered support from Katrina Murray Community Mental Health Nurse who would visit at home. Fiona declined this support at this time but would prefer to liaise with Graham Cameron.

Graham reported the dietician has said there is not a problem with Jessie's weight loss.

Alison McPherson (Staff Nurse) reported there have been no problems with Jessie's skin folds or problems with her bowels.

Family reassured that support is available and encouraged to keep communication open.

Agreed at meeting Jessie could be discharged same day.

Graham Cameron to organise carers - 2 in the mornings, to start Wednesday 27 October 2010.

First of all, this is just nitpicking, but I assume it's "Gayle" not "Gaye" (though I could be wrong). Second, I thought you weren't meant to mix written numbers and digits? - E.g. "Six" and "6".  Third, I hate when people don't add the suffix to a number within a date. "27" instead of "27th", etc.

In other news, it would appear I'm nothing more than a Grandson. Despite the fact that I'm the one getting carers allowance. Granted, I didn't introduce myself as anything more, but apparently the minutes keeper didn't take down my name at the time any way, just goes to show how little they know about me.

If, as noted, the dietician said there were no problems with her, the Staff Nurse said there were no problems with her (the district nurse has also said the same thing for months), why was ol' 'Gaye' even invited to the meeting? When, as stated in the previous blog, Graham knows next to nothing about the case at all, having not even talked to the previous person in charge, or, it would seem, having read the notes.

It should also be noted that 'Gaye' came into the meeting late, and was not introduced to the group. Nor were we introduced to her. A seemingly important person to not be introduced, no?

Also. Last thing. "Jessie was admitted to Muirton Ward six weeks ago." -- It was eight weeks!

Edit: I just noticed. "2 in the mornings, to start Wednesday 27 October 2010" -- False.

Only one carer came in, and they didn't start coming in 'til November 3rd.
